What to Bring
- Valid picture ID
- Social Security numbers for all persons listed on the tax return (including dependents)
- Birth dates for all persons listed on the tax return
- Prior year’s tax return and supporting documents
- For direct deposit of your refund, bring a voided check OR your checking or savings account number and routing number from your bank
- Income statements and needed forms (if applicable)
- Wages from each job (Form W-2; all copies)
- Independent contractor earnings (Form 1099-MISC)
- Interest earned (Form 1099-INT)
- Dividends earned (Form 1099-DIV)
- Stock sales details (Form 1099-B)
- Retirement Plan Distributions (Form 1099-R; all copies)
- Gambling Winnings (Form W-2G; all copies)
- Unemployment (Form 1099-G)
- Social Security Benefits (Form SSA-1099)
- Railroad Retirement Benefits (Form RRB-1099-R)
- Financial Broker Year End Combined Statements
- Hobby Income
- Income expenses and needed forms (if applicable)
- Medical/doctor/dentist expenses paid out of pocket
- Health, Medical account contributions (Form 5498-SA)
- Health, Medical account distributions (Form 1099-SA)
- Proof of Health Care coverage (Form 1095-A, 1095-B, or 1095-C)
- Insurance premiums paid out of pocket (not through employer pre-taxed)
- Long-term care premiums
- Prescription drugs paid out of pocket
- Medical miles driven
- Estimated tax payments made (amounts and dates paid)
- Real Estate Taxes (include copy of county’s statement)
- Other taxes paid: personal property; vehicle tabs (MN only); etc.
Major purchases with sales tax paid - Home mortgage interest (Form 1098)
- Other mortgage interest (Form 1098 – need to explain use)
- Charitable contributions cash or check: need receipts with amounts listed
- Volunteer miles driven
- Noncash charitable contributions: need receipts with amounts listed
- Gambling losses
- Unreimbursed employee expenses
- Union dues
- Previous tax preparation fees
- Hobby expenses
- Safe deposit box rental expense
- Investment expenses (paid out of pocket)
- Receipts for education expenses you paid for your children in grades K-12. (Ex.: tutoring, rental/purchase of an instrument, music lessons, pens, pencils, notebooks, etc.)
- Tuition expenses you paid to attend a university, college, or technical school (Form 1098-T) and expenses (Ex.: notebooks, books, lab fees, laptop, etc.)
- Interest you paid on student loans (Form 1098-E)
- Day care expenses you paid for your children. (Need provider’s name, address, and tax ID or social security number)
- Renters: Certificate of Rent Paid (Form CRP) from each landlord for the year
- Anything else you believe to be helpful or necessary in completing your tax return
